- Complete New Patient Forms (downloads below)
- List of ALL Medications you are taking
- Insurance Card and any applicable insurance information
- Driver’s License or valid photo ID
- Completed Consent Form (required for Parent/Legal Guardian of Minor patients)
- X-Rays or MRI Results related to your injury, if available
- Appointment Confirmation Make sure to confirm your appointment date and time before your visit

What Can I Expect?
For many people, visiting a chiropractic physician is a completely new experience. You may have heard about chiropractic care from a friend, read about it online, or simply reached a point where living with pain is no longer an option. Whatever your reason, your first visit is focused on getting to know you—your health history, current condition, and personal goals for recovery.
At West Coast Spine Center & Professional Physical Therapy in Sarasota, your initial appointment will include:
Completing any necessary paperwork (or bringing your new patient forms if filled out at home)
A one-on-one consultation with the doctor to discuss your symptoms and health concerns
A thorough examination to determine the underlying cause of your neck pain, back pain, or other musculoskeletal issues
X-rays, if needed, to rule out serious conditions and help us design the most effective treatment plan
After your examination, we’ll review your results in detail and recommend a personalized treatment plan, which may include Gonstead chiropractic adjustments, physical therapy, laser therapy, or auto accident injury care.
If you already have recent X-ray or MRI results for a specific injury, please bring them with you to your first appointment.
Same Day Treatment
Once your examination and any necessary diagnostic studies are complete, most patients receive their first treatment during the same visit. This may include precise Gonstead chiropractic adjustments, targeted physical therapy, and/or soft tissue therapies to address pain, improve mobility, and promote healing.
Home Instructions
Before you leave, you’ll receive clear, personalized instructions for activities and care at home. This may include guidance on applying ice or heat, avoiding certain movements or positions, and performing recommended home exercises or stretches to support your recovery between visits.
Schedule Your Next Appointment
We will schedule your follow-up visit at a convenient time—typically within 1–2 days. During this appointment, you’ll receive a complete review of your exam findings, along with a detailed discussion of your treatment options. Our goal is to keep you on track toward lasting relief from neck pain, back pain, auto accident injuries, or other conditions.
